Insurance & payment

We bill most major plans directly. You’ll see your portion in writing before treatment — never as a surprise at checkout.

  • Photo ID and insurance card (if using benefits)
  • List of medications and allergies
  • Recent X-rays if you have them — we’ll request transfer if not
  • Arrive 10 minutes early for forms on your first visit
